1. Data controller
1.1. The controller of your personal data is Altima Dental Horban, Redka Spółka Jawna, with its registered office in Kraków (address: ul. Starowiślna 82/4, 31-035 Kraków), entered in the Register of Entrepreneurs of the National Court Register by the District Court for Kraków-Śródmieście in Kraków, 11th Commercial Division of the National Court Register, under KRS number: 0000531563, NIP: 6762481840, REGON: 360152538 (hereinafter: the “Controller” or the “Clinic”).

2. Purposes and legal bases of processing
Your personal data is processed for the following purposes:
2.1. Handling enquiries through the contact form and online chat: we process data (e.g. first name, surname, telephone number, e-mail address, message content) in order to answer your questions and arrange an appointment. The legal basis is the legitimate interest of the Controller (Art. 6(1)(f) GDPR), consisting in communication with patients, as well as taking steps at the request of the data subject prior to entering into a contract for the provision of medical services (Art. 6(1)(b) GDPR).
2.2. Provision of medical services (where health data is supplied): if you provide us with information about your state of health through the form or the chat, we process it for the purposes of diagnosis and the provision of health care (Art. 9(2)(h) GDPR in conjunction with the Polish Act on Patients’ Rights and the Patients’ Rights Ombudsman).
2.3. Analytical and marketing purposes: to analyse traffic on the website, optimise its operation and display personalised advertising. The legal basis is your voluntary consent, given by accepting cookies (Art. 6(1)(a) GDPR).
3. Data retention period
3.1. Data from the contact form and the chat is stored for the period necessary to handle the enquiry, and thereafter until any potential claims become time-barred.
3.2. Where an enquiry results in treatment being started, the data becomes part of the medical records and is stored for 20 years (in accordance with Art. 29 of the Act on Patients’ Rights).
3.3. Analytical data is stored until consent to cookies is withdrawn or the cookies are cleared from within your web browser.
4. Recipients of personal data
4.1. Your personal data may be transferred to entities processing it on our instructions, including:
- The hosting provider: Hostinger, on whose servers the website and e-mail are maintained.
- Providers of IT tools supporting the operation of the Clinic.
4.2. Analytical and marketing tools (transfer of data outside the EEA): the website uses tools from external providers. As a result, data may be transferred outside the European Economic Area (EEA), including to the USA. Such transfers are based on standard contractual clauses or European Commission decisions. We use:
- Google Analytics (Google Ireland Ltd.) – to analyse website statistics.
- Meta Pixel (Meta Platforms Ireland Ltd.) – to analyse and target advertising on Facebook and Instagram.
- TikTok Pixel (TikTok Technology Ltd.) – to optimise advertising campaigns on TikTok.
- Microsoft Clarity (Microsoft Corporation) – to analyse user behaviour on the website (so-called heatmaps).
5. Rights of the data subject
5.1. You have the right to:
- Access your data and obtain a copy of it.
- Rectify (correct) your data.
- Erase your data (this does not, however, cover data contained in medical records, which we are required by law to retain).
- Restrict processing.
- Object to processing.
- Data portability.
- Withdraw consent (to the extent that data is processed on the basis of consent) at any time.

5.3. You also have the right to lodge a complaint with the supervisory authority – the President of the Personal Data Protection Office (PUODO) in Warsaw – if you consider that the processing of your data infringes the GDPR.
6. Voluntary nature of providing data
Providing data in the contact form or in the chat is voluntary, but it is necessary for us to be able to reply to your message or arrange an appointment at the Clinic.
